Position: Nurse

You will work alongside the already existing nursing team. Your primary role as first point of contact with a variety of patients is to provide them with first rate care and attention.
 
You will typically be providing practical nursing care i.e. checking temperatures, blood pressure and respiration rates, assisting doctors with physical examinations, giving drugs and injections, cleaning and dressing wounds and administering blood transfusions and drips. Much of your work is likely to involve using a mixture of hi-tech and traditional medical equipment.

Essential Responsibilities

  • Observe and assess patients' progress, changing the care plan if necessary, in consultation with doctors.
  • Assess, plan, implement and evaluate individual treatment plans for patients with a known long-term condition HIV/AIDS
  • You'll also have daily interaction with patients and their relatives.
  • Ensure clear understanding and utilisation of referral mechanisms within the practice
  • Assist patients from marginalised groups to access quality care be culturally sensitive and increase access to health care for all
  • Participate in the local implementation strategies that are aligned to the values and culture of general practice
  • Accept the rights of individuals to choose their care providers, participate in care and refuse care.
  • Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions
  • Work with patients in order to support adherence to prescribed treatments
  • Support patients to adopt health promotion strategies that promote patients to live healthily, and apply principles of self-care
  • Implement and participate in vaccination and immunisation programmes for both adults and children
  • Meet the needs of patients presenting for opportunistic wound care


Requirements

  • Registered Nurse with Clinical skills –i.e. immunisation and vaccination, e minor surgery
  • Experience working in a clinical environment where adults and children receive emergency, urgent and/or unscheduled care
  • Demonstrably able to work in an interdependent manner with all other professionals and clinicians
  • Previous experience working in an outpatients/surgical setting
  • Knowledge of needs of patients with long-term conditions
  • Change-management skills and ability to support patients to change lifestyle
  • Awareness of clinical governance issues in primary health care
  • Knowledge of health promotion strategies public health issues in the local area
  • Ability to identify determinants on health in the local area
  • Aware of accountability of own role and other roles in a nurse- led service
  • Ability to communicate effectively both written and verbal including communicating difficult messages to patients and families
  • Negotiation and conflict management skills IT skills
  • Ability to work core hours
  • Teamwork, Self initiative, Enthusiasm and Flexibility i.e.  covering other shifts
